Osteochondral lesions of the talus typically present as cartilage denudation after trauma or cystic in bone with intact cartilage. fractures require an anterograde approach for treatment. Retrograde drilling and packing a graft can treat Because subchondral involve posterior aspect talar dome, these be difficult to localize image intensifier. We describe method which ankle arthroscopy is used visualize lesion, along ACL jig target lesion precisely. : performed retrograde two patients: 29-year-old man 34-year-old man. Each patient presented posteromedial talus. treated both patients by modifying standard technique drilling. Patients were evaluated before surgery using AOFAS score X-rays joint. The taken at 3-month follow-up showed well-maintained contour dome complete incorporation graft. scores greater than 90, excellent per Saxena Eakin criteria. This decreases radiation exposure, invasiveness, surgical time procedure.
